Creating a Titular Empire title

Hey guys I'm struggling to find out how to create an Empire titular title. Every time I create the decision the game gives me the green 'tick' to confirm the action is doable but when I click it nothing happens. Any ideas why it doesn't work? If it helps I'm trying to create an empire from a kingdom tier title.

I tried to do something similar through an event.

This event works, so maybe you can distill from that how the decision is supposed to look.
