Its happened a few times to me now, when I create custom kingdoms, if I destroy the kingdom (when I make an empire) it gets marked as inactive and I can no longer create it, BUT all of its de jure land remains there.

Although, if it gets destroyed by the "De Jure Requirement" Rule, it does not get marked as Inactive (from experience).

There are a lot of weirdness with generated titles that haven't been addressed.
The affected titles are custom titles created by decisions, nomadic realms and clan titles, dynamic titles (such as those created by religious liberation, crusades and the like)

Another problem is that if they've got no holder, no claimant and no de jure vassal they will disappear from the title history leaving a buggy interface behind them:
