Just got the game. Played the normal game for a bit to get to grips with how everything works and now have the mod installed.
Are there any huge differences in game mechanics that I should know about before playing? I'm aware of things like the megawars and extra events/decisions which are additions to vanilla, but is there anything that has drastically being changed? For example how you usurp/create titles for duchys?

Well theres a new map…

One big difference (in my opinion) is the dueling system. You may notice some characters, such as Robert Baratheon and Jaime Lannister, has traits that have something fighter (poor fighter, trained fighter, skilled fighter, formidable fighter). In game, characters can face off in duels, where these traits, and others such as strong, affect the outcome of the duel. It normally ends with one dying or yielding. These duels are most common during battles (akin to Robert vs Rhaegar in the books) or in trials by combat (bronn vs ser vardis in the books)

To create a duchy, you most personally hold one of the counties in it and be the vassla of or owner of at least half of the total counties. I'm not sure why this was changed, i guess it was done so duchies that dont exist in the books (such as the shield islands) arent created instantly. Also in the mod they are called high lordships.
 
Other than things that are the mods own creations, such as the dragons, colonisation and duel system for example, there aren't too many changes other than the duchy creation one and the fact the vanilla rebel faction system is not used.
 
For the creating titles that doesn't seem like a major difference from vanilla. What about claiming high lordship titles via usurping, is that the same rule as creating?

Yes, one pretty huge one - mega-wars and vassals that function on a better level than in the vanilla, and actually choose depending on relation and loyalty whom they'll back (or even stand by).

This is the only radical change that I can think of and one you need to grasp before playing (that different vassal system; improved in my opinion and with hope PI will pick something up from it) the game.

Also the way combat functions has been altered (a lot, I think).

There's a heap of tiny ones. E.g. the dueling system, dragons as sorts of superweapons, and a vastly increased importance of traits and titular titles.
