Vassals go independent with no warning

1.02 CK

If you are attacking, say, a 10-province pagan Kingdom and, to avoid demesne penalties, creating Counties as you go (all of them with 100% loyalty), as soon as you take the last province of that Kingdom, all your new vassals go independent. Modifiers: disbanding all the vassal regiments just before the last province falls does seem to decrease the likelihood of that happening.

There is no way a 100%-loyal vassal can break away in a snap, is there?

There is, if they manage to get enough land & gold to create a title equivalent as yours...

I've had the same problem.
And there is NO way that any of those vassals had the same gold, land, or titles that I had as King of Denmark (etc) and King of Rus (etc). You give them their land and they seem jolly happy, but then suddenly they are no longer your vassals. This is obviously a bug.

I also encountered this problem, and I think I already read about it on the forum, although I cannot find the corresponding threat right now.

The problem occurs if you conquer land from another realm and then grand titles that where vassals of your enemy. When you destroy your opponent, all his vassals, including those that are already yours(!), become independent.

As this explanation is not really good, let me given an example:
You are in a war with the Kingdom of the Fadmids, who have most of the Holy Land as vassals. Let’s say you conquer Jerusalem and create the County of Jerusalem. Then you move on to Egypt and completely destroy the Fadmits. Then, all the sheikdoms (= counties) which were their vassals become independent, including the Count of Jerusalem because the Sheik of Jerusalem used to belong to the Fadmit's realm.
Also, this problem does only affect existing titles. If you create the Duchy of Alexandria (from Fadmit land) it will stay your vassal after the Fadmits are destroyed.
(Still not a clear description  )

In the related threat (the one I cannot find) someone suggests that this is due to the fact that sheikdoms and counties have the same tags, and I think it is already recognized as a bug.
