I changed the House name of one of my vassals, and then the dynasty tree was messed up; it seems that the new house founder appears as the dynasty founder

It is a bug for sure, but it has nothing to do with you have renamed the House, but because the Game doesn't like Cadet Branches House Head to become Dynasty Head, the Game only wants the Founder House Head to be Dynasty Head.

You can get this Bug yourself, if you create an Cadet Branch and you are more powerful than the Dynasty Head.
