Each country has its own leader/monarch file, with ID for the leader. There is also a HistoricalLeaders.xls file in the Leaders folder. What if anything should be done with the .xls file if you want to edit and add your own leaders? The leader files for the countries and for HistoricalLeaders.xls don't seem to match, but I could be wrong.
 
Just by using a text editor like Notepad in Windows, and following the format already used in the file. The only thing to be careful about is that sometimes the file will be saved with a text extension, so leaders.nip will end up as leaders.nip.txt for example. Easily renamed, you just have to be aware of it. Oh, and avoid rich text format or any other fancy things.

If you are preparing leaders to add to the IGC, there are ID numbers: every monarch or leader must have one, and there are a limited number. There is a sticky thread to tell you which ones are left, and you can claim them there in advance of using them so noone else's clash with yours.
