My MacBookM3 pro, has worked alright up to now. All of a sudden I am experiencing extreme slow startup times. I have deleted mods, reinstalled twice, updated MacOS, deleted saves.
What can I do?
 
At a risk of being a captain obvious, every time you update the game or change mods, your startup cache may get invalidated and the game may need to regenerate shaders. Some steps, like compiling shaders takes A LOT OF TIME. So if you didn't do it, try to run game twice without actually changing anything and see whether second startup is smoother.

Sadly no change whatsoever. Really frustrating
 
Did you delete the game by means of Steam? It only deletes the files that are known to Steam, which not necessarily contains the whole game folder.

If you haven't tried emptying the folder manually (and doing the same with the game-related subfolder within Documents), this could help.
