Just purchased the Gangs DLC after a year or so since my last playthrough.

Set up my first new prison and have 4 huge bugs:
  1. It's always dark, no matter what time of day the clock says
  2. Staff walk through walls. Thought this was a little helper for when you're first setting up your prison, but they continue doing it even after prisoners arrive.
  3. When the first intake of prisoners turned up:
    1. They all have massive heads
    2. They are all gang members (I set Gangs to 'Low' when creating the prison, so this feels very wrong. Prisoners are Med & Low security)
  4. Also, not sure this is a bug but I don't remember it being like this when I played before: fog of war is insane - every room I don't have staff in is completely obscured, including furniture, floors and even walls. Most of the map is a grey blob. (Not playing on Warden mode, by the way.)

If I recall the list of mutators correctly, all of these effects are settings toggled by a mutator. I'd double-check which mutators you have on.
You're right, that was it - thanks. I guess the bug is that about a dozen mutators got applied to my new game even though I never even opened the mutator menu?
