I first loaded up the game, patched to 1.06 in Steam this morning, and found that for some reason, the map is covered by horizontal rows of triangles and rectangles. They persist across map modes, from the main menu into the game itself, and, when I zoom down as closes as I can, they follow the contours of the landscape (rising and falling over hills). I've reinstalled, had steam check the fragmentation and presence of the game files, activated and deactivated all my DLC (SoI, African Units, Mongol Faces, Extra Crests) and nothing seems to change. Any idea what could be causing this. I'm playing on a relatively ancient graphics card (ATI 4300 or so) on a decent computer, and have never had an issue like this before. Help?

That does look like a video card or driver problem, but try a full clean reinstall: uninstall the game from Steam and move any valued save games elsewhere. Manually delete the CK2 directories from under the Steam AND MyDocuments folders, now reinstall the game via Steam.

If that doesn't help, please post a dxdiag.txt (you can delete everything after the "Sound devices" section for brevity)

Looked for it and couldn't find it, then tried another full reinstall, deleting everything and reinstalling from the ground up. No change.
Just to be clear: you deleted everything in BOTH the Steam/CK2 and MyDocuments/CK2 folders?

We have seen this issue before, I now remember. And both those people with the ATI 4500 card like you.

One of them solved it with a complete video driver/catalyst reinstall, so I suggest you try that. Uninstall your video driver (so using the MS default) and Catalyst, reboot, then install those again fresh.

Well, I tried uninstalling everything driver-related, including Catalyst and all, but unless I did something wrong, nothing seems to have changed. I'm still seeing lines of triangles. Fortunately, this isn't the end of the world, as I should be getting a new video card some time this week.

This happens because ATI 12.4 drivers have some issues. You need to install a hotfix and it will clear it up. Heres a link for the hotfix. http://support.amd.com/us/kbarticles/Pages/AMDCatalyst124bhotfix.aspx

AMD transfered some older HD graphics cards to legacy status and so ended up with buggy release. This effects people using ATI Radeon HD 2400, 2600, 2900, 3400, 3600 and 4500 graphics cards. I myself had this problem and installing the hotfix fixed it.
