They once showed excellent aptitude, no more.

i have two herculean women in my court, they have terrible aptitude after I loaded my savegame

I was unable to reproduce the issue, both on 1.11.5 and on 1.12.1, From what I can see Court Physicians doesn't increase the Aptitude on either versions, at least when starting a new game. Do you have a save game from 1.11.5 where it did increase their aptitude? And are you sure the game is unmodified?

No mods. I was told herculean helped with the aptitude for the court physician but maybe its not meant to?

I'm sad to say that you have been given misleading information. The Herculean trait does not increase the aptitude of court physicians.
