Hello there! The present is a design choice and not a bug. When your Pronoia is disloyal it will not annex after its ruler dies. It also remains Pronoia so when they next ruler dies loyal – it will annex. If you have any questions about it, don't hesitate to ask about it on the main forum for the game.

I have the same issue. I've converted Bulgaria in a non-hereditary Prononia, and after their ruler died, I haven't inherit. Now, I'am stuck with a Bulgaria with a desire liberty of 100%, and full of heirs. With no option con convert again in a non-hereditary Prononia. Is this a issue o normal in the game?

It is normal that you don't inherit disloyal Pronoiars. You have to get them loyal if you want to inherit them

Even after the dead of the ruler, I can't inherit. I decrease the desire liberty below 15% each time. It had happened three times until now, and nothing. And every time that Bulgary has a new ruler its liberty desire rises at 100%, with all of my rivals backing up its independence.

Can you attach a save game in which they are loyal when their ruler dies and don't get inherited? Why does their liberty desire rise when their ruler changes? Maybe that's preventing the inheritance

Just had this happen to me, with Serbia. They had a Royal Marriage with me that they initiated, so they were at ~47% liberty desire before their ruler died and 55% after. They didn't get inherited. Savegame of the aftermath attached.

If would say that this is to be expected. If they initiated the royal marriage, it ends when their ruler dies and then they lose the -5% liberty desire from that and so they are above 50% liberty desire when the game checks if they should be inherited. And they also lose the +25 opinion from the royal marriage which can lead to another increase in liberty desire if they are not at +200 opinion afterwards.
