How do counter measures work?

I've played for only a little bit, but one thing I can't figure out is launching counter measures. Eithre from planes or ships. I watched as SSMs hit my ship had full chaff. I clicked it but nothign happens.

What am I doing worng?

Flares, chaff, and torpedo decoys are launched automatically, although it seems like sometimes they won't launch.

Countermeasures always make one automatic last ditch attempt. There may be a problem with updating the flare/chaff count used, but it always triggers when available.
