How to capture other country?

I am using Austria in 1499. I declared war with Milan, and then beated her. When I sieged and then captured the city. The computer said I have gained the control of Milan.
However, the Kingdom of Milan never disappeared. When I checked the info of Milan, it said it is controlled by Austria but 'own' by Milan. Milan send thousands of peace letter to me. I declined all of them.
The problem is how I can eliminate 'Milan' and take over its land???

You're probably playing with the forced annexation option off. Check your game settings.

Or did you actually offer them peace ? If you do so with annexion on you should get an option to annex them if you've captured all of their land...

But even when the option to annex is there, the other party may still decline, remember that. This often happens when you are fighting an alliance.
