Ok, I used to know how to do this, but lately I have been trying to do this, andi t is not working. Therefore, can someone make a tutorial for me showing how to mod these three things?

The easiest way to learn is to see how other modders have done it. A quick summary:
governments - in common folder plus you need a localisation (plus you might need ruler localisation), then an entry in the history/countries/nameofcountry.txt file
titles - in common folder plus you need a localisation and a graphic and an interface file adjustment
bookmarks - in common folder plus a localisation entry
