Hey there. I've been playing the game for years, though never getting any HRE outside of the Empire.
Today I've run into the EU2 guide: [ http://www.vpltd.com/freedownload/IGEU2.pdf ] that says in the chapter 7.14 [page 31] that it is absolutely possible for a country outside of Empire to become a Holy Roman Emperor, it just needs to satisfy the conditions [greatest sum of positive relations with all the elector countries]

Have you ever experienced a HRE being, to say - Poland or any other country from outside? Can you confirm that it's possible?
 
Yes, it is possible, but only for Catholic/CRC countries (Spain in particular seems to get it fairly often). Protestant and Reformed countries may become emperor only if they are inside the empire.

Have seen Bourbonnais become HR Emperor lately what seems quite off-key but well whatever the whole campaign is ahistorical anyways
 
Take a skim through the "Strange Screenshots" thread[s?], and you'll see plenty of "outside" Emperors. Catholic Incas have become HRE, though that's obviously a rare case.

In my experience, situations like that will happen later in the game, when the number of Electors (and potential electees) diminishes greatly.

I'm not sure of the exact event chains involved, but I know Spain & Austria both get it a lot because of the Habsburgs. I feel like the inheritance of Burgundy and the Low Countries has something to with it as well. I think that whichever country (Spain or Austria) inherits them will continue to become HRE for quite a stretch. Though that could be the reverse.
