For the most part issues appear to show up when you can actually do something, but a handful can appear even when you can't use it.

For example if you crown authority is too low to imprison characters it still shows up as an issue.

To add to this maybe give options to things that are hard to accept. Such as having your player character become obese suddenly (you just have to accept it) and then dying a few years later due to "complications" maybe we should be given some stressful options, or something, to do something about it?

Another event that comes to mind is my player character being given a very ominous warning by a mysterious religious person and then mysteriously dying a few months later. I feel like I should have been able to do something about it instead of being handed a "bad luck you lose" card.

I think that event is supposed to be similar to the one that lets you know you'll die in a year. I think it's supposed to be giving the player a heads up to your death not a punishment cutting it short. I might be wrong though.

I agree otherwise though!
 
The game tells you you can declare war against someone who is clearly more powerful than you and you have no chance of winning against and that's somehow an issue that you can't make go away from the issues tab lol
