1. Why do vassals with a high opinion refuse to participate and sit out neutral?

2. Why is there no penaltys for a vassal not fulfilling his feudal obligations and sitting in his keep?

And why are small vassals under kings like an independent Robb or Vassals for Renly while he is backing Stannis, and still a lord paramount, sitting out?

Ther can be many reasons for a character sitting out, it could be due to alliances or religious/cultural differences. The point is opinions are not the only factor.

As for penalties I guess you could argue for them, maybe make it a just reason for imprisonment. But then if you look at the lore examples, the Freys, Lannisters, Martells, Arryns, Swanns, none of them had many serious consequences for sitting out. The Martells are even courted by the Iron Throne, being offered a royal marriage.
