I don't think this has been fixed properly in 1.03a.
I haven't seen any rulers with two claims on the same title but I have seen many with about a dozen claims on the same title.
It's fairly common. In my game that's reached 1140ish there are examples all over the place. I've got a save game and a couple of nice screenies that illustrate the problem.
