I would assume however that the HRE would be quite upset to see his (at least theoretical) crown of Italy slip away.

If something like this happened in real life, I would expect the HRE to try to retake Italy and then "reform" the Papacy as an institution under the HRE's direct "protection" (read "control").

depends on what you mean with an eastern pope of course. There've been plenty of popes from the Byzantine regions (syrians and so) untill fairly late.
