First of all, I want to thank Shaytana (even though he's gone) and everyone involved with this mod up to now. I discovered it only a week ago, yet it's simply breathtaking. The work, the beauty, and the alternate history itself it's like he read my mind. Everything is there, it's just mind-boggling. As a History major, I really appreciate all the though put into this, all the intricacies of scenarios, new and old, colliding.

So, to the question itself, as a complete noob: I started my first playthrough as the Dux of Dalmatia, descendant of Diocletianus, and by now I control the de jure kingdoms of Illyria and Venetia and the dukedom of Ferraria. My question is, how do I move forward once I get empire-worthy (and grab Rome itself from the S.P.Q.R.)? Do I create my own empire like Vanilla? Do I restore the Western Empire? Can I usurp S.P.Q.R.? There is some kind of decision involved?

The S.P.Q.R. is the Western Empire for gameplay purposes. The decision to restore the "true" Roman Empire from vanilla is something that's been planned to be integrated for some time now. So yeah, at the moment your best option at restoring Rome is usurping the S.P.Q.R. and the Eastern Empire.

For some reason I'm getting more and more frequent CTDs during play now, any clue on how to fix that?

The common cause of CTDs in LI is too frequent autosaves. So if you're playing in Ironman or with yearly autosaves you're going to have more crashes the further you advance in the game. I'd recommend setting them to every 5/10 years or disabling them and just remembering to save manually, and the crashes should be a much less frequent ( still there probably, though ).

I know it's a bit of a nuisance, but there's little we can do towards alleviating it.
 
Guess I can try that, but funnily enough, I never actually had a CTD during autosave.

It doesn't exactly crash during the autosave, but at the moment the game is going to autosave.
