Well it's obvioussly kinda dark and it's not like it happened every time a city was taken but there are accounts of rulers/generals putting cities to the sword during this time period. Some examples are corinth which was destroyed by the romans and gaza had it's inhabitants put to the sword by the jews.

Either make a 4th option when taking a city and make it even more devastating or have more dynamic events akin to when you conquer rome/carthage/pella/athens etc for normal countries and what to do with their cities.

Beyond the obvious effect, what is in for the player? They did it to make an example to other powers to be and to quell future rebellions. In the game this fear is not simulated (it is not loyalty nor happiness).

A big loyalty boost region wide, show the locals you mean it. Should last a while. If that game had some proper colonization and population control mechanics this would also be a good opportunity to replace the populace with one more suited to your needs.

Yes, replacing the POPs you kill with integrated culture POPs from your nation would be something that a player may want to do. This will be only possible if you own the city, not during a war and during the first year after being conquered by an army on the city.

The loyalty boost could be an unrest bonus for 60 months in that province/region?
