I was playing 1.03a as Glamorgan, later Wales, and Rome at some point was conquered by Islam.
At that point the game stopped appointing popes and papal controllers.
However, on a reload, it announced that the person with the highest piety (a duke in Scotland) was the papal controller. He had no cross by his name, and nothing else obvious occurred.
Later I had a CTD for no particular reason. These things happen. I went back to my last save, and discovered that now I had the highest piety, and was so declared Papal controller in reload.
However, I didn't have the cross. I looked at my options, though, and sure enough, I could excommunicate. I went to excommunicate my neighbor, and bam! CTD.
It seems to me that
a) there should still be a pope even if Rome is conquered;
b) The Christian AI should go nuts if Rome is conquered by Moslems or Pagans, and try to take it BACK; and
c) if for some reason there is no pope, there also shouldn't be a papal controller, and excommunicate should be greyed out as an option automatically.
