Okay, other than having them at the start of the game and having someone take one of your national provinces, how does a permanent CB occur? Can you inherit one? I am playing England and I periodically go over each countries relations by clicking them on and I noticed that Poland-Lithuania had a perm CB against me. Huh? I have never fought a war against them, never been in an alliance with them that I broke, in fact my relations is relative high - 120+. So Why? The only thing I could think of was that I recently annexed the Hanseatic League. Could Poland have had a permanent CB agaisnt the HSA which I inherited?
 
Poland regards some of the Hansic provinces as their "heart provinces" - their right. A country has a permanent CB towards any country owning one of "their" heart provinces (your heart provinces are those with the english shield upon - when you look at the political map (you can't see other countries heart provinces this way)).
Often different countries can regard the same province as their right.
 
Just to expand on the core province issue, the CB follows
whomever owns the other country's "core province".

Example playing Venice I watch Bavaria grab Tyrol and Styria
from Austria. Austria has permanent CB on Bavaria. Taking advantage of an opportunity I DoW Bavaria (several years later) and take Styria from Bavaria. However as a core Austrian province, Austria now has a permanent CB against me (Venice.)
 
A heavy problem with minors is to have a cb. As Brandenburg, I did not have one at the beginning. So, when I declared war against Bohemia or Hannover, I did not have CB (ouch!) so stab. dropped to 0. But I came back to +3 in a year!

Now, Hannover and Austria have a perm. cb against me :) (Bohemia is consider by Austria as national province. Sorry Austrians, I annexed it...)
