This is talking about CK-DV, no other mods as yet.
I don't know about creating titles, however, you can play any country you want if you put the following into the scenario.eug file, for example with Khazars,

The part of the .eug file which has C101 etc and all those four-letter kingdom and duchy names is the one which determines whether it's a playable country. That title is the one which will appear on the selectable screen.
"type" is county, duchy or kingdom, whatever is the highest title owned by the character.
"dna" is just the image for the starting screen when you first select the country and it doesn't affect play at all - or what the character looks like in play.
"title" is the kingdom and duchy titles held by the character, and the CXXX is the county number - so you have to know all the counties run personally by the king/queen or duke/duchess. So you have to know the province numbers, and the abbreviations [
edit: you can discover them for DV, which changed the country codes from CK vanilla, in C:\Program Files\Paradox Entertainment\Crusader Kings\config\world_names.csv and C:\Program Files\Paradox Entertainment\Crusader Kings\config\extra_text.csv]. This makes this system easiest to use with small countries, it'd get difficult later on.
When you load the game up with the text above, it lists the Khazars among the duchies but for some reason refers to the character as "
King of the Khazars, Count of Itil." Then in play he's still just a duke.
When you save the game, it will wipe that little bit of text out. You can edit the save.eug file, too, in the same way as the scenario.eug, and voila it's a playable country again. Thus you must edit the save.eug each time you want to load it up and play.
So the method suits long sessions of play, not endless reloads.
I've just started experimenting with this today, so I don't know if it'll allow us to create or grant titles. As Khazars, there was one potential title offered - King of Khazaria, which was 61 provinces or something, I wasn't going to slog through that to find out.
I'll report more as I experiment more, unless others have their experiences to share and save me the trouble
