4. When did this start happening?
new game yesterday after destroying the ship yard its stuck. One of the 3 fleets spawning after the first battle spawned in ai controlled system and was destroyed by ai. But quest continued to destroy the shipyard. But situation log does not show it after destroying it.
I analyzed the debris to get Lance weapon progress but it's not shown as available research option (too early in game?)
Im playing as barbaric despoiler + life seeded with authoritarian and fanatic militaristic.
5. Can you replicate it, if so, please explain how
Not yet only happened in this game
6. Does this happen on all saves or just specific ones?

7. What steps have you taken to try and solve this yourself (reinstall, Google error code etc)?
Can't reload prev save in Ironman.
