what are they used to in the game ? are they needed ? how to modify them ?

They are used for events, like calculating if a province is to far away to be ruled effectively or for the English Melting Pot event.

I never modified them, but my guess is that you can modify them in Crusader Kings/db/provinces.csv
 
any influence on spreading of culture religion etc ?
 
Generally they have some influence, but not major one except for the melting pot events.
AFAIK there is no map for CK 1.05 or DV. There used to be an older map for patch 1.04, but the regions have changed in 1.05
I vote you to fix that. :p

Regions and areas can be modified in Province.csv, for DVIP I made one small modification: I moved a province in Asia Minor that belonged to a unique region not used anywhere else to it's neighbours's region.

Modifiying regions and areas on a larger scale requires also modifying all events that use it...

It's pretty, but are those the 1.04, 1.05, or DVIP regions?

The way North Africa is treated is perhaps responsible for all those Moroccan DOWs against Nubia, but weird the Balearics join in...
 
DVIP, which is identical to DV and 1.05 with the aforementioned one province change.

Regions and areas don't have any effect on DOWs as far as I can tell, nor on any other game mechanic. They're only used in events.
Morocco DOWs Nubia often because it has a free land connection to it (all the other NAFs are coreligionists of Morocco), and Nubia is a very weak realm.
