Running Designer's Cut, I captured a ship and, in the ship-capture dialogue, assigned it to its own fleet with a new captain. That new fleet appeared on the strategic screen, but the original ship and fleet vanished altogether. (My only frigate, too!)
Just downloaded the patch -- don't see this problem on the list, but I hope it was addressed.
