There is a event that can be triggered normally or via completing a mission from the Tibetan mission tree that gives the option of adopting theocracy. Hovever, the government will be an ordinary theocracy, with standard theocratic heir, not quite fitting the whole Dalai Lama reincarnation concept.

I suggest giving Tibet a unique tier 1 reform, giving it a different system. You would not get the standard heir events. Instead the government would get the same effect as the Nahuatl religion, where regencies can declare war. Every time a lama dies, an event would pop-up giving you a regency council High Lamas with 2/2/2 (or perhaps 3/3/3) stats. Within a year or two you would get an even about High Lamas finding the reincarnation. The new Dalai Lama would obviously, be a kid. If the Lama somehow dies before coming of age a new event about the reincarnation being found wuld happen.

The whole system should not require much effort, one government reform and 3-4 events, but it would make playing as Tibet more flavorful.

What conditions would invalidate and revoke this reform? Changing religion, culture, tag, or any of the 3?

Would this be a locked tier reform? Would it prohibit switching government type?

It's not as simple as you think. Not a bad idea though, would be nice to have something special for Tibet.

It should be limited to the three Buddhist religions or even just Vajrayana. I wouldn't bother making it locked, just a replacement tier 1 reform, probably with a small bonus to devotion, instead of +5% tax of Clerical State, to compensate for the lack of selecting from multiple heir options. You either accept the Lamas with all the good and the bad, or you go wherever your heart leads you.

That's a pity (it not being simple), but I hope it may one day be added. It feels a bit weird to play Tibet with all the unique mission tree and a then discover that going theocratic gives you an heir system that does not fit at all. Although I can blame noone but myself, as the theocratic heirs were part of my own suggestion years ago.
 
I have been planning some suggestions for tibetan government reforms to add to my larger tibet overhaul post.

Since theocracies are fairly weak due to their shorter reform chain, I wanted to create some reforms with unique flavor for tibet making for more unique playstyle as a theocracy

The choosing heir system could be improved, but I don't consdider it the most important, in part because the Ganden Podrag system were a fairly latecomer in tibetan history. Before that monastic or quasi-theocratic states were typically inherited by male line relatives.
