My reward for reestablishing the HRE, and getting a REALLY good heir set up with adoring vassals for the future...

...is to inherit next to nothing, because I formed Italia in order to keep my realm united. The decision to reform the is supposed to make all fully-controlled kingdoms outside of Italia de jure HRE. My previous ruler controlled all of Italia seen above. His heir inherited Burgundy and Provence.

Well, you did that to yourself then, buddy. Just play it out and reconquer your lost territory. That's what Ironman is supposed to force you to do.

A couple things.
The dejure empire for East Francia is Germania which the HRE effectively replaces, could you go to the dejure view and post a picture of that as this is not an issue I myself have ever seen where Italia expands.
If you controlled West Francia, Frisia and Lotharingia even if they were only vassals they should be included in your current realm. It does not make sense for them to have split off in this way.
What succession type or laws did you have in place?

The decision to reform the HRE deletes the empire of East Francia, and makes all fully-controlled kingdoms outside of Italia de jure HRE. The problem is, when it destroyed East Francia, it defaulted to the already-existing Empire of Italia instead of the HRE.
Let your heir inherit Italia then.

I think I found the problem. From Reddit:

"What others have said about Italia, but also an important note: I believe the tooltip in the game is misleading (it used to be) – it says it'll add kingdoms whose territory you completely control to de jure hre (except for those from empire of Italia), but instead it adds kingdom titles that you hold to hre, no matter if you control them fully or not. So, like, if you fully control Bulgaria but didn't create the title, it won't be added to HRE. But if you managed to create Bulgaria title with whatever amount of counties, and don't control it fully – it will be added to HRE."

The only kingdom title I formed outside of Italia was Burgundy.

Unintuitive/unexpected behavior is different from rolling with the punches from normal gameplay, it's fine to complain about them or to force quit the game to roll back to a previous state, now knowing how some mechanic or decision actually works.

But yes, I assumed this was fixed with recent updates, but apparently it wasn't:(

The tooltip says "completely controlled kingdoms are added to de jure HRE", but the game doesn't care about completely controlled kingdoms, instead it adds kingdom titles you hold (regardless of how much of their territory you control – and as long as they don't belong to e_Italia) to newly created HRE.
Another bit of misleading info from the tooltip is "kingdom of East Francia will be destroyed": actually, all the kingdom titles you have will be destroyed (if you have any vassals in e_Italia but haven't created empire of Italia yet – be prepared for the "not rightful liege" debuff or be prepared to recreate Italian kingdoms or create empire of Italy).

P.S. shenanigans like that and the fact that the game is still very unpolished and basically in development despite being out for 5 years is why I personally never play in Ironman (since the achievements are available anyway).

Who creates the holy Roman empire when they could've just unified Italia and go for recreating the actual Roman empire?!
