SO I found A LOT of discussions on this topic.
I read through all and NONE worked.
Here is what I did to fix my issue:

* Step 1 *
- Navigate to error.log:
= C:\Users\<your user>\Documents\Paradox Interactive\Crusader Kings II\logs
- Check the eror logged within
= "...Could not find file /gfx/flags/flagfiles_0.tga ..."

* Step 2 *
- Navigate to local folder of crusader Kings 2
= C:\Steam\steamapps\common\Crusader Kings II\gfx\flags

* Step 3 *
- Check file names of files located within this folder
= See that file name are "d_palestine.tga"; "d_vestlandet" etc.

* Step 4 *
- Rename all flagfiles to "flagfiles_X.tga"
= Replace "X" with incrementing numbers from 0
= I did this with an auto renamer app
= You can do this with a batch script or manually
= There are 2.7k files to rename

* Step 5 *
- Once all files are renamed run game again
= Game boots into main menu
= Game also runs fine now

Do that in the steam folder and all your renamed flag files will go bye-bye next time Steam updates CK2.

Do any and all changes in a mod folder. (I assume your problem stemmed from one mod or another, as vanilla CK2 does not try to find flagfiles_X.tga)

(P.S.: Register your game so you can access the user modifications subforum.)
 
...you might as well have told Steam to verify the file integrity, and it'd have 99.9% likely have solved it in one relatively simple operation.
 
I deleted the gfx/flags/ folder, verified integrity of game files. Steam re-downloaded the files with the old names i.e. not "flagfiles_X.tga"
Which is why I resorted to renaming them myself.
 
Why was CK2 looking for flagfiles_X.tga if you didn't have any mods? It looks for <title tag>.tga for flag files. (example: k_england.tga for the flag of the Kingdom of England.) You've done something screwy that you aren't telling us.
 
Well I don't know what to tell you, I installed the game via Steam, without any further action after the installation, booted it up. Then it proceeded to load up until "Processing Flags" then crashed.

I tried booting it up again, same thing.
I checked the steam community forums for solutions, I found:
* Delete /gfx/flags folder (Did that, didn't change a thing)
* Re-install direct X and VC Redist. (Did that same error)
* Run the game in computability mode (Ran it first in XP SP3 mode nothing changed and then in Win7 mode, nothing noth times again with admin rights, same error)
* Re-install the game (I wanted to play it that same night with some friends so I didn't do that)
* Update graphics drivers (I started downloading my drives)
* While the drivers were downloading I ran it a few more times while it kept crashing, so I figured I'd check the error.log, ig it even had one.
Which it had under the C:\Users\Myuser\Praradox.... folders and within that file it gave an "File Not Found" error indicating it was looking for flagfiles_X.tga.
* So only then did I proceed to do the batch renaming thing.

Please tell me where the "screwy" thing is that I did.

Since I have no way of further proving this is what I did, you're just gonna have to believe this is all the steps I took.
Believe me or don't because this is what I did and nothing more.
 
If you have indeed done nothing screwy, then register your game and post in Tech Support sub-forum. There's something screwy with your system.

If you don't know how to register the game, click on "link your account to Steam" at the top of the forum main page.
 
Whilst it *does* look for flagfiles_0.tga and so on, they're not in the steam apps folder.
They should be in \Documents\Paradox Interactive\Crusader Kings II\gfx\flags

And there are 3 of them
 
That would be the flag cache. He says he deleted gfx/flags, but I think he must have deleted steam/steamapps/common/crusader kings ii/gfx/flags rather than documents/paradox interactive/crusader kings ii/gfx/flags
