Unable to Game Over with no colonists - possible interaction with Pheonix Project

Please explain your issue is in as much detail as possible.
General description:
After attempting to "Game over" for ("into the unknown" achievement
Killed all colonists by turning off domes
Because of "pheonix project" breakthrough, they have a 50% chance of coming back to life, making it a slow attrition
It seems the "dead" colonists' homeless status and unemployed status remains;
After the remaining colonists leave on asteroid, there are "0" colonists and 200+ homeless, unemployed colonists
Expected behaviour:
When a colonist dies with "project pheonix" 50% chance they are instead changed into a youth with the same perks, quirks, flaws and specialisation
Actual behaviour:
When a colonist dies with "project pheonix", the original death cleanup is not complete, leaving a "zombie" homeless, jobless colonist
Can you replicate the issue?
Possibly - have not repeats aas it is a late-game issue requiring a specific breakthrough
Reloading save and repeating the kill-off / send away still dowsn't trigger game over

As per extra testing and replication below, there is an issue with the "game over" status not triggering (needed to get the achievement I was hunting),
There appears to be a second issue with "phantom" colonists being not counted in population totals, sometimes with a "ghost"location, and sometimes unable to be located by the game.
Although the phantom colonists may prevent the gameover status, they are not required and the two are independant
I continue to suspect that colonist death handling process and the trigger for gameover status may be currently bugged
