How do I install a second with an older patch without having to uninstall everything first?

I do not think that you can. The registry key would "tell" the CD that you already have the game installed.

If you are worried about the saved games that you have then move the save game file and rename it. You can then move it back after the reinstall - the same goes for virtually any other file you may want to retain from the one you are going to uninstall.
