Yes. There are four possible things that may need to be changed. One always needs to be changed, one will need to be changed if you are giving away the primary province from a character, and three are always optional although one is strongly recommended.

  1. (Required Always) Find the title associated with a province, C123 or the like (always C + province number, always 4 characters long), and edit the id of the owning character to the id of the new owner. This is in a title = { block that contains just one tag listing and a holder = { block.
  2. (Required Sometimes) If the province edited away was the primary title of the previous owner (search for a country = { block that lists the tag of the province you just transferred away, C123 or similar), you will need to either a) delete the country entry and all characters assigned to that country (if that was the only province that country had) or b) change the country entry and all characters assigned to that country to be another province belonging to the transferring character.
  3. (Optional, Recommended) Edit the liege of the county title moved (the liege = line in the title = { section with the holder) to be the liege of the receiving character, or ---- if the receiving character is independent.
  4. (Optional) Edit the list of titles held by both the giving and receiving character in the top of the save file in the first title = { section. This is only used for display of ownership at country-selection, though.
  5. (Optional) Edit the controlled = { list of provinces in the giving and receiving characters' country = { entries at the bottom of the save file.

The end result is that it's very easy to edit secondary provinces from one character to another without shifting the control entries (which will reset after 1 day in the game if the realms in question are not at war) and a royal pain in the ass to edit primary provinces from one place to another.
