What requirements are there for the kingsmoot to happen? I played a game as ironborn and forced the elective succession in the iron islands through faction demand, but even after several kings died the kingsmoot never happend. Instead it just went with the standard elective succession. Does the kingdom need to be completely independent?

I think so, yes. I also think you need to have the empire tier Iron Islands title rather than the kingdom tier one.
