Mercenary companies are Open Elective. I imagine that had an impact.

Or for some reason they gained a city first before grabbing the duchy title (race condition maybe?)

Yes, with the console. No, it's not really viable--you get a free army while you're unlanded, and once you're landed you have to pay upkeep for them, which is staggeringly high. Other characters can hire your mercenaries, but you don't get any of the money they pay. You also can't get ducal or higher titles without using the console.

A nice guy like that should NOT be pope. Popes, Kaisers, and Mongols are the three things that I loved beating the living daylights out of in this game.
