2 things CK1 did not have were twins (or triplets or more). They were implimented in Rome and I think they would be a good addition to the game here moreso than Rome.

In addition, CK1 had several possibilities when a female gave birth: success, stillborn or death and the child dies. It did not have the 4th option, death and the child survives.

Good points, I wouldn't mind seeing that.

I'd also like a kind of Alexander IV-style regency, if the King dies with a pregnant wife, it could be a boy. That changes the game somewhat.

In that situation I would almost hope for a civil war, because otherwise.. if they institute a regency.. you'll be waiting for a long... long... time. :D

I think it would depend on the country. Civil war, yes in the case of Barbaric nomads, but I'd say no in the case of say a Western European country, most of the times. Thats what happened to the last main Capetian line, before the Valois(lesser capetian branch) inherited. One of the queens was prego, and it wasn't decided until she gave birth to a girl.
 
The possibility of a civil war could be affected by the legitimacy rating..
