This may be related to this bug, but has happened to me in a different situation. Occasionally, a vassal duke of mine seems to abruptly lose his vassalage status to me and become independent. I was not at war with anyone, it was not a newly granted title, the duke's loyalty was very high, he did not rebel/declare war, the ruler was the same as the last 20 years (i.e. it's not a case of a new ruler from somewhere else inheriting). Basically, there seems to be no rhyme or reason to why this happens - one day he is just no longer my vassal.
