In my game as Sparta, whenever one of my characters gets the ambition of wanting a title. If you try to give them the title on the same day as they get the ambition it will not let you (greyed out). Check again in subsequent days and that character is completely missing from the pool. He will reappear again when he changes his ambition, which quite often is another title.
EDIT: I've also noticed that if you view a characters dynasty page, hover the mouse over the blank area above their job and quite often the tooltip will read "Wants a title". Monarchy characters do not have the same blank grey box that republic chars have when they want a title.
