As the USA, I have seen this happen:
Event:
Senate Committee investigates shipbuilding in [New York]
Selected: A Critical weakness
Started a dockyard in New York (2 available slots)
2 day later, Two Ocean Navy Act COMPLETED.
5 slots generated with 5 dockyards. 1 DY under construction with 6 slots undeveloped.

Arsenal of Democracy started (which would add 3 slots and 3 dockyards). this would still be below 25 slots
After the dockyard completed construction, fulfilling the event, no Completion in the decision panel and no bump in Congressional support, which is at 79H, 284S.

I have seen this happen before, but it was a MIL in Utah.

I suspect that due to multiple foci that added the similar construction to the same state (New York) along with construction a flag was set and not cleared since the Focus dockyards do not fulfill the event, and the constructed dockyards surrounded the event.
 
It's a bug that's been there forever. It's specially annoying because Congressional support is an essential part of playing USA.

It's not clear what circumstances trigger it. I don't think it's related to in-progress or completed Focuses. It appears to be random. It can happen more than once in a campaign, with either MIL or DY. Building another MIL/DY in the same state, after the previous one failed to register, before the timer runs out doesn't work either.

Someone else in this forum suggested a while ago to delay the construction of the requested MIL or DY by at least one day. That seems to have worked completely in my last USA campaign where I followed that advice from day 1. It may have simply been luck. Who knows...

Try this in your next USA campaign (or the remainder of your current campaign):
- Accept the event
- Don't act on it immediately
- Let time pass so the event countdown decrease by at least 1 day (you can do other stuff during that time, of course)
- Start construction of the requested MIL or DY

It takes some getting used to but if it is indeed an effective solution, as it seems, it's worth the extra mental effort so as not to get distracted and completely forget about it.
