I was playing a game as the Kingdom of Portugal and at war with Badajoz. Suddenly, Badajoz has changed to Galicia and the war has ended. My enemy (Ya'far III) has died and is succeeded by his son Yusuf.

That is the heir to the throne there 3 years ago, my last savegame. I was already at war with Badajoz at that point. The only thing that changed was Badajoz won a claim war against Toledo, expanding its territory to what it is now (as far as I can tell). It was still called Badajoz after that.
Galicia was created by Ya'far III at June 17th 1208, the day he won the claim war, and then inherited by Yusuf.

This looks like a bug to me. Yusuf had no titles until he inherited the whole bunch at November 1st 1208 (I only can't check with the barony, because it doesn't track history). There is no reason why the war should stop when the defender dies and does not become part of a bigger realm after that.

I've had a similar issue where an ally and I were attacking the emirate of Valencia and then the king of my ally died and he pulled out of the war. When he pulled out half of the warscore went away because he had been doing most of the seiging at the beginning.
