Since no steam code existed for it, I suppose that it could not have had an expiry.

Yes, 500K icon was given to everyone registered when forum membership hit 500K. Same for 200K one, but I think you had to participate in 200K Sale as well, because not all members of the time got it.

And about icons not showing up - now you can choose what to show and what not in settings.

Yes, for 200k you had to participate in the sale. I'm number 52235 but I don't qualify...
