You are forced to control at least one of the counties in the duchy directly to create it. It's annoying when you want to organize your realm.

It fits the lore though. Kings don't just declare themselves Dukes. Powerful counts do.
The problem is that the game isn't very stable character/land wise. While this makes sense in the actual lore and GOT universe... in this game it doesnt. 50 years in and titles disapear and land falls into the wrong hands by accident.
