I agree with colombo that you can't rely on patrols to take out ladders... unless you make a whole bunch of berzerkers and manually put them in every nook and cranny of the hallways in your dungeon. Then you have to manually set those buggers to patrol where they'll wonder off in a general radius and attack ladders and heros. The problem I have with this "strategy" is that there is no real way of maintaining these patrols as they all look alike and I might accidentally tell one them to do something else ruining my little patrol chain. So in a way, there is no rational way of dealing with ladders better than by just stopping what your doing, such as having fun, and teleport on top of them.
 
To be honest, I think multiple entrances would be a better choice than ladders. With ladders you are pretty much force to take care of them immediately, since they can spawn right next to your treasure chamber. There is one idea however, that could allow an effective way of protecting treasure chambers though, a trap that you can assign one squad to and have that squad teleport to the trap when activated.
 
Vast majority of his issues are resolved by proper strategy and runes/upgrades.

My favorite complaint is the comparison to Dungeon Keeper having an entrance: Bad news everyone, heroes didn't use one entrance in DK either. They busted through your wall, anywhere they can get to you.

The heroes will *always use the entrance* if you destroy the ladders. Every single time. With proper design you can completely prevent the ladders from ever being a problem with 1-2 patrolling units.

I've watched Total Biscuit's WTF's a lot, and while humorous, unfortunately they're usually about as accurate as the people who can't figure out basic game mechanics.

In part this is true, but do not forget that in DK you could fortify walls.
 
TB summed it up pretty well.

From what I can tell from the demo, it's a something of a cross between DK and Settlers. You have direct control over military units while civilians / imps do their own thing.
There is also an RPG element where Baal himself can evolve into one of three uber units :D
It's a pretty good prototype game :D
