There is a lot of things on a screen I am not familiar with.

Probably the crystal nidus glitch that's been seen by just about everyone at this point. It's not really supposed to happen, but the devs left it in to see what else people could catch.

Anyway, here's a few screenshots from the start of my latest playthrough:

Never seen a megastructure one system away on day one before. I guess we'll never have to wonder whether we're alone in the galaxy...

For the next one, the neighboring fallen empire finally gave me open borders, so I thought I would check their black hole system to see if it had an L-gate:

I guess the good news is that entering the system made it hostile with the fallen empire, so that's one more system I can claim... eventually.

That is not an L-gate!
It is a gate to the "Long giant monster" Dimension.
So, close.

Fun fact: if the big guy is the only fauna in your Viva, you are treated as if you have no fauna.

The big guy is probably bugged (or just not handled) because optimal player behavior was to kill it for the hull plating tech.
 
If you truly want to know:
I found 44 names in the REP3.txt Nameslist.
Getting any name once is 1/44, or 2.2727% recurring.

Getting the same name thrice is 1/44^3 or 1/855184. So, a bit shy of "one in a million".
However that is the current namelist (3.99.3 beta to be precise). You are using a older version (pre 2.0?), so the list might have been longer or shorter.

Sigh. Yes, but there are 44 names on that list so there are 44 different outcomes where all 3 names are equal. So it is 44/44^3 = 1/44^2.
To put it differently, Pick any name.The chance to get it on the 2nd slot is 1/44 and the chance to get it on both 2nd and 3rd slot is 1/44^2.
But the chance to get any name on the first slot is 1.

getting one name is x/x = 1
Getting second time same name is 1/x
Getting third time same name is 1/x
Now multiply it

Maybe this is normal, but first time I've seen a purifier end up with some other empire's species as their leader

Absolutely not normal. They should be blocked from any getting any leaders of another species.
Maybe if this is a artificial leader like the Oracle? But that one should be blocked.

I would definitely report this as a bug.
