Since Italy often leaves the Central Powers, it would be aesthetically unpleasing to have the Italian flag next to Germany's and AH's.

I know it's aesthetical.
But Triple Alliance was born under the consense of these three major powers, as much as happens in HoI standard. The Axis powers are these 3 powers: Japan, nationalsocialist Germany and fascist Italy.
My Italy ( when I used to play in 1936 -> 1948 ) always become a superpower with powerful tanks division ( 150mm cannons :D ) so I've never saw the event Repubblica di Salò on North and Badoglio's Monarchic Italy on South. But at the end the Italian flag remains.
In this case it's a little different for Italy wins the war, but if you see in this perspective ( Triple Alliance - Italy, A-H, Reich Germany ) Italian flag must remain in the winning screen.

And some more:

624 Agram (Zagreb)
743 Spalato (Split)
747 Ragusa (Dubrovnik)
739 Temesvar (Timisoara)
615 Prag (Praha)
731 Hermannstadt (Sibiu)
759 Kronstadt (Brasov)
730 Klausenburg (Cluj-Napoca)
740 Esseg (Osijek)
619 Austerlitz (Slavkov)
729 Nagyvárad (Oradea)
612 Pilsen (Plzen)

Have also changed Rijeka from Pola to Fiume.

Historically, did Italy join the Entente powers?

Yes.

Case closed. :)

Well if you please, then change the history.
In my point of view, I look at the Triple Alliance as a treaty born under the consense of these three european powers ( I actually don't care if you say that Italy wasn't a military and colonial power as other European powers, the fact is that in Europe the major power were England, France, Reich Germany, Austrian-Hungaric Empire, Russia and Italy ). That is the historical fact.
Turkey join the alliance later, and even if Italy quit in 1915 to join the Triple Entente, still I see the Triple Alliance in this focus: simply a treaty made by these 3 powers.
If you say then Italy often quit to join Entente ( I don't know how it is called in english, if we say Entente we refer to the first treaty, instead "Triplice Intesa" means Triple Entente ) and so cannot figure even in the final victory screen in Entente flags, then do so.

More cities:
Ulan bator = Urga
Kirovabad = Jelisavetpol
stalino = Juzovka ( I think, will spellcheck later, this requires a namechange of donetsk though, since it is the same city)

And, from the bottomscrape of the infamous barrel, I´ve picked up some more names:

Cluj: Klausenberg
Timisoara:Temesvar
Muscat: Masqat
Sibiu(if is is in the game): Hermannstadt
Stalinabad: Djusambe

Mitilini should be Greek, as it was under their occupation in 1914.

This was confirmed by a declaration signed by Britain, France, Russia, Germany, Italy and Austria-Hungary in 1914 which gave Greece sovereignty over Lemnos, Samothrace, Lesbos, Chios, Samos, Ikaria and a load of other small islands in the region.
