What I think happened was that Venice was already in a war against someone else when Milan declared war, and that someone forced Venice to give Milan Bergamo in their peace treaty, while Milan were already at war for Bergamo.

If the AI has a bunch of favors built up it can request relative as heir even if neither side is christian. It doesn't really do anything for non-christians since they don't get PUs but they can still do it, so probably when that guy came to power Janpur requested relative as heir with favours
 
Common misconception is that an heir has to be a child of the ruler. It easily could be an uncle or older cousin if the ruler has no legitimate children.
I know that heir doesn't have to be a monarch's child (and know some historical instances too), but didn't know that heir can be older bcs well older people are more likely to die first

On investigating more closely, it seems like Austria got a female ruler which forced the electors to pick someone else - and because catholicism, despite being the 'dominant' religion of the empire, is actually pretty weak, there evidently weren't a lot of eligible in-empire candidates. By which I guess I mean none. Currently, the runner-up for the next election is Brittany.

...you know, Polish separatists, that isn't really much of a change.

Other goofy things in this game that neither I (Hungary) nor player Venice caused:
  • Yet another 3-dev remnant Novgorod (this one in Karelia)
  • England getting fully occupied by Scotland while trying to conquer an Irish kingdom... and then annexing said kingdom
  • Sweden has had 100% liberty desire and plenty of supporters for a century, but never actually got around to declaring independence (it's 1552)
  • The Ottomans have never attacked the Mamluks
